We offer home care and supported living services aimed at enhancing the quality of life, dignity, and independence for individuals and families. Whether individuals need a few hours of support or full-time care with personal tasks, learning new skills, or becoming active community members, we have a compassionate care team to support them. Our person-centred approach anchors everything we do at Case Management CHC. We understand that each person is unique and has different needs. We work with individuals on a personal level and involve them and their families in the care process as much as possible.
